| ** Examination fees must be current prior to Testimony. Minimum due and payable prior to court / deposition.
Standby fee will be reduced to $50 per hour if notified prior to noon that services will not be required. |

| Written report includes brief summary of the examination and conclusion / opinion.
An expanded report delineating specific findings upon which the opinion is based is available on request and will be billed at the
Administrative Services rate. |

| Terms: All fee balances for examination are due and payable,
in full, upon completion of the exam, concurrent with issuance of the report / conclusion and/or invoice, unless prior
arrangements have been made. ReBeL reserves the right to retain possession of the written report and/or client's
documents until account is settled in full. If billing arrangements are made, invoices not paid within 30 days of the invoice
date will be subject to a late charge of 1 1/2% per month on the unpaid balance. Invoices unpaid after 60 days are subject to legal action
in which case the client will be held responsible for all legal and/or attorney fees, collection charges, and/or court costs in connection therewith.
All verbal contracts are made with legal counsel. Contracting counsel will be held responsible for payment of any and all services and/or
associated expenses / costs. |
